Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Clinical Social Worker (LCSW) to work onsite in one or more long-term care facilities within your local community, providing therapy, assessments, care planning, and behavioral health support.

This role offers professional autonomy, a predictable Monday–Friday schedule, and robust administrative and clinical support to help you focus on helping residents thrive.

Qualifications

• Active, unrestricted LCSW license.
• Ability to work independently within an interdisciplinary team.
• Strong written, verbal, and interpersonal communication skills.
• Must have valid driver's license and reliable transportation.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments and treatment planning.
• Support residents with depression, anxiety, and other LTC-related conditions.
• Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate care.
• Document using the GNT and maintain confidentiality and compliance.
