Job summary

Deer Oaks Behavioral Health is seeking a licensed clinical social worker (LCSW) to join our integrated clinical team in long-term care facilities within your local community. You will provide therapy, assessments, care planning, and behavioral health support for older adults and medically complex residents.

Qualifications

• Active, unrestricted LCSW license.
• Valid driver’s license and reliable transportation.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, and other LTC concerns.
• Deliver behavioral management and goal-oriented interventions aligned with geriatric care.
