Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Specialist Clinical Social Worker (LSCSW) to join our integrated clinical team onsite at long-term care facilities in your community. You will provide therapy, conduct assessments, and contribute to care planning to help residents thrive, with a predictable Monday–Friday schedule and strong clinical support.

Qualifications

• Active, unrestricted LSCSW license.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Comfortable navigating LTC environments.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, and behavioral issues.
• Deliver behavioral management and brief, goal-oriented interventions.
• Work with nursing, social services, rehabilitation, and medical staff to coordinate care.
• Participate in care plan discussions and facility meetings.
• Maintain complete documentation using Geriatric Notes Tool (GNT) and ensure compliance.
