Job summary

Fox Rehabilitation in New York is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes and senior living communities. You’ll assess, plan, and treat to improve mobility, independence, and quality of life under our Geriatric House Calls model.

Clinicians enjoy autonomy, flexible schedules, and opportunities to incorporate telehealth or virtual care to extend access while maintaining high-quality outcomes for older adults across the communities we serve.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
