Job summary

Fox Rehabilitation in Middletown, NY is seeking passionate, licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes and senior living communities. You’ll evaluate, treat, and develop individualized plans of care, helping older adults improve mobility, independence, and quality of life through our Geriatric House Calls model.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Document care in FOX’s EMR system following regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate.
