• MD or DO degree required and board eligibility/certification in family practice, internal medicine, geriatrics or palliative care
• Current unrestricted State medical license
• Reliable transportation and valid driver's license/insurance

Responsibilities

• Provide advanced primary and specialty medical care to geriatric and medically complex patients, often in patients' homes or care settings
• Provide medical leadership and supervision for NPs and PAs; coordinate care with families and interdisciplinary teams
• Improve patient outcomes, reduce hospitalizations, and mentor clinicians
