Centra Health is seeking a qualified Social Worker to provide home health services, helping patients and families access community resources and coordinate care in their homes.

The role requires an MSW and at least two years in case work or counseling within a health care or social delivery setting. A valid Virginia driver's license and proficiency with MS Office and EMR are preferred.

Formación

• Master of Social Work (MSW) required.
• Minimum two years of case work or counseling in healthcare or social delivery settings preferred.
• Proficient with community resources for geriatric populations and able to navigate referrals.

Responsabilidades

• Assess caregiver capacity and develop care plans with the patient.
• Assess payer coverage to inform transitional care planning.
• Conduct interdisciplinary assessments and develop individualized plans with patient and family.
• Provide counseling to patients, caregivers and families.
• Educate patients and caregivers on care and resources.
• Communicate with physicians and other health professionals regarding home health plans.
• Support quality improvement activities and discharge planning.
• Demonstrate empathy and negotiate transitions of care and service levels.
• Maintain knowledge of regulatory, accreditation and payer guidelines.
• Document accurately and maintain chart integrity.
