Job summary

VNS Health seeks a licensed social worker to assess clients’ psychosocial status and social work needs. You will establish the social work component of the patient/family care plan, coordinate referrals to community services, and deliver direct service during home visits and care transitions.

Responsibilities include counseling, resource planning, crisis intervention, and active participation in discharge planning to ensure seamless care across the care team.

Qualifications

• Master's Degree in Social Work from a graduate program accredited by the Council on Social Work Education.
• Licensed to practice Social Work in New York State (Education Department certification/licensure)
• Minimum of one year of social work experience in a health care setting.

Responsibilities

• Assess clients and family psychosocial status and social work needs.
• Establish the social work component of the patient/family plan of care and make referrals to community services as needed.
• Travel to patients' homes or care facilities to deliver direct service.
• Provide short-term counseling, community resource planning, and crisis intervention as required.
• Participate in discharge planning and coordinate care with the health team.
