Job summary

CenterWell Home Health, part of Humana Inc., seeks a Medical Social Worker to evaluate and develop care plans addressing emotional and social needs of home health patients. You will supervise a Social Worker Assistant and work within multidisciplinary teams to ensure compliant, patient-centered care.

Responsibilities include assessing patient and family needs, coordinating with community resources, and documenting care activities while adhering to company and regulatory standards.

Qualifications

• Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.
• Social Worker licensure in the state of practice, if required by state law or regulation.
• A valid driver's license, auto insurance, and reliable transportation.
• Proof of CPR certification.
• Experience with discharge planning needs and obtaining community resources.

Responsibilities

• Assess the patient's social and emotional state related to illness or injury and care needs.
• Assess relationships between medical and nursing needs in home setting and available resources.
• Coordinate with community resources to resolve barriers to recovery.
• Communicate with patients, families, and care teams; provide ongoing assessments.
• Maintain documentation including conferences, visit reports and progress notes.
