Job summary

Masonicare at Home in Wallingford, CT is seeking a Home Care Services Specialist for a part-time, day-shift role. The position oversees care plans, coordinates with clients and families, and ensures safety and quality in home care services.

You will travel within CT, assist with intake, train staff, and maintain documentation, reporting to the Executive Director. Bachelor's degree in a health-related field preferred; private duty homecare experience is valued.

Qualifications

• Willing to travel throughout CT for majority of the workweek.

Responsibilities

• Serve as liaison between clients and care teams to coordinate plans.
• Create and update individualized care plans with families and professionals.
• Provide guidance to caregivers and support staff, escalating to leadership as needed.
• Handle intake, referrals, and follow-up.
• Conduct home care assessments to ensure safety and satisfaction.
• Maintain accurate documentation of visits and communications.
• Participate in community events to promote Masonicare at Home.
• May assist with recruitment and interviewing of care staff.
• Communicate effectively with diverse clients and staff.
• Travel across CT for the majority of the workweek.
