Job summary

The PacificSource PACE LLC is seeking a Licensed Dietitian (LD) to deliver nutrition interventions for older adults enrolled in the PACE program. You will be part of an interdisciplinary team that coordinates dietary services in the Day Health Center and collaborates with primary care providers to optimize quality of life.

You will assess nutrition needs, develop care plans, provide education to participants and families, and document care in the EHR in a compliant, timely manner.

Qualifications

• Minimum 2 years of experience in clinical nutrition, community nutrition, or foodservice management.
• At least 1 year of experience working with geriatric or frail populations.
• Experience in PACE or other Long-Term Care settings preferred.
• Must hold Oregon LD credential; CPR required.

Responsibilities

• Conduct comprehensive nutritional assessments and develop individualized care plans.
• Participate in IDT and provide clinical nutrition expertise for team planning and problem-solving.
• Advise participants and families on dietary plans and healthy eating.
• Coordinate menu development and work with food vendors to meet participant needs.
• Accurately document in the EHR and ensure HIPAA compliance.
• Collaborate with physicians, nurses, social workers, and therapists to provide integrated care.
• Attend team meetings and participate in program committees.
