Job summary

PacificSource is seeking a PACE Licensed Dietitian to join our team in Oregon. You will work with PACE participants, provide nutrition interventions, and coordinate dietary services within the Day Health Center, collaborating with primary care providers to optimize quality of life.

You will conduct assessments, develop care plans, educate participants and families, and document in the EHR, while following state and federal regulations. Travel ~20% of the time.

Qualifications

• Bachelor's degree in Dietetics or related field required.
• Oregon LD credential required.
• At least 2 years in clinical or community nutrition; geriatric experience preferred.
• Certification as RD/RDN or CDE preferred; CPR and driver's license required.

Responsibilities

• Conduct comprehensive nutritional assessments and develop care plans.
• Participate in the Interdisciplinary Team (IDT) and coordinate dietary services.
• Provide nutrition education to participants, families, and caregivers.
• Inspect meals for diet conformance and assist with menu development.
• Document in the EHR promptly and comply with regulatory requirements.
• Collaborate with physicians, nurses, social workers and therapists to deliver integrated care.
