The PacificSource PACE LLC is seeking a Licensed Dietitian (LD) to deliver nutrition interventions for older adults enrolled in the PACE program. You will be part of an interdisciplinary team that coordinates dietary services in the Day Health Center and collaborates with primary care providers to optimize quality of life.

The PACE Licensed Dietician (LD) is a mission-driven role focused on improving the health and quality of life for older adults participating in the PACE Program. The role of the LD is to function as a key member of the Interdisciplinary Team (IDT) and work with PACE participants to provide nutritional interventions and education. The role coordinates the dietary services of the Day Health Center and works with participants of high medical complexity in collaboration with primary care practitioners to optimize quality of life, while honoring what matters most.
A minimum of 2 years of experience in clinical nutrition, community nutrition, or foodservice management is required. At least 1 year of experience working with geriatric or frail populations, is required. Experience in PACE or other Long-Term Care, Services, and Supports settings is preferred.
A Bachelor’s degree in Dietetics, Nutrition, Food Systems Management, or a related field is required. Candidates must hold an Oregon Licensed Dietician (LD) credential. Certification as a Certified Diabetes Educator (CDE) is preferred. Nationally recognized credentials as a Registered Dietician (RD) or Registered Dietician Nutritionist (RDN) is preferred. A valid CPR certification is required. A valid driver’s license is required.
Employee(s) must have the ability to work effectively with individuals who have diverse health issues, concerns, and personalities. They should be proficient in accurately documenting case notes, assessments, contacts, and writing letters that may become part of legal records. Excellent organizational skills and experience with various computer software applications, including Word, Excel, PowerPoint, and Outlook, are recommended. The capability to work independently with minimal supervision is required, along with exceptional communication skills, both verbal and written. Strong active listening skills are also necessary.
Each member of the PACE organization’s staff that has direct contact with participants must meet the following conditions:
Work inside in a general office setting with ergonomically configured equipment. Travel is required approximately 20% of the time.

Work is performed in a clinic setting with typical healthcare exposures, including contact with bodily fluids, infectious diseases, and cleaning chemicals; PPE is provided and required. The role involves stooping, bending, sitting, standing, and moving between clinical areas for extended periods. May assist participants with mobility, including guiding, supporting, escorting, and occasional transfer assistance using proper body mechanics. Must be able to lift, push, pull, or carry up to 35 pounds occasionally and perform frequent repetitive motions such as typing and operating medical or office equipment. Some positions require occasional travel to PACE sites, participant homes, or external appointments. Must be able to speak clearly, understand verbal instructions, and interact professionally with participants, caregivers, and staff.
