Job summary

BoldAge PACE, based in Fresno, CA, is seeking a Registered Dietitian to assess nutritional status and plan nutrition interventions for PACE participants. You will work closely with the Interdisciplinary Team to ensure nutritional needs are met and educate participants and families on dietary issues.

The role emphasizes collaboration with IDT, development of care plans, and ongoing nutrition education. A bachelor’s in nutrition/dietetics and CDR exam are required, with 1 year elderly care

Qualifications

Bachelor’s degree in nutrition, dietetics or related field required.
CDR exam passed required.
Minimum 1 year working with elderly population; training provided if not.
2+ years clinical nutrition experience preferred.
Serve Safe certification preferred.

Responsibilities

Assess nutritional status of PACE participants and plan nutrition interventions.
Provide nutrition counseling and education to participants and caregivers.
Coordinate with Interdisciplinary Team to develop care plans.
Monitor weight, hydration, intake, and food-related issues; report changes.
