Job summary

Trinity Health is seeking a Registered Dietitian/Nutritionist to support the PACE program for elderly participants. You will perform nutritional assessments, develop personalized care plans, and guide dietary adjustments to promote independence and well-being.

Join a multidisciplinary team, provide professional education, supervise dietary aides, and coordinate with meal providers to meet each participant's dietary needs.

Qualifications

• Bachelor's degree in food and nutrition, dietetics, or related field.
• Registered Dietitian/Nutritionist licensure.
• Minimum one year of experience working with frail or elderly populations.
• Strong knowledge of clinical nutrition and therapeutic diets.
• Excellent interpersonal skills and ability to collaborate effectively.

Responsibilities

• Conduct initial and annual comprehensive nutritional assessments for elderly participants.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Monitor participant progress and recommend dietary adjustments.
• Provide education and training to healthcare professionals as needed.
• Oversee dietary aides and ensure adherence to standards of care.
• Act as a liaison with meal service providers to ensure dietary needs are met.
