Job summary

NeighborHealth's PACE program seeks a Registered Dietitian to join the interdisciplinary team. You will perform clinical assessments, develop care plans, and coordinate nutrition interventions to support participants’ health and chronic condition management.

You will oversee nutrition competencies for dietary staff, provide culturally appropriate nutrition counseling, and collaborate with the Nutrition Manager to maintain clinical excellence and adherence to policy.

Qualifications

• BS or MS preferred.
• At least one year as a Registered Dietitian.
• RD eligible or RD from an accredited institution.
• State licensure within 3 months of hire.
• Maintains state and national registration/licensure.
• ServSafe Certification including MA Allergen Training.

Responsibilities

• Completes nutrition assessments and implements care plans at enrollment and annually.
• Updates progress notes and care plan problems and interventions.
• Provides chronic care management based on person-centered goals.
• Addresses high risk nutrition issues including weight loss and skin breakdown.
• Collaborates with the IDT to align goals and plans of care.
• Provides culturally and age-appropriate nutrition counseling.
