Job summary

InnovAge Holding Corp. seeks a Dietitian to assess and modify participant nutrition plans, deliver education to participants and caregivers, and ensure home or facility-based service delivery is effective.

You will work within InnovAge PACE to support care plans and participate in cross-functional teams. Responsibilities include developing Plans of Care, monitoring dietary needs, and coordinating with IDT to adjust meals, supplements, and nutrition education.

Qualifications

• Bachelor’s degree in food science, nutrition, and/or dietetics from an accredited college/university is required.
• Current medical terminology and the ability to complete written assessments and document participant care.
• A minimum of one year experience working with the frail or elderly is required.

Responsibilities

• Completes participant nutritional assessments and reassessments and plans appropriate interventions.
• Develops nutritional Plans of Care and reassesments every six months or as needed.
• Provides nutrition education to participants, caregivers and InnovAge PACE staff.
• Maintains nutrition notes in medical records and communicates with IDT.
• Supports meal services and participates in interdisciplinary teams for care plans.
• Monitors dietary intake, therapeutic diets, and coordinates resources like Meals on Wheels as needed.
