Job summary

InnovAge is seeking a Dietitian to complete participant nutritional assessments, plan and implement nutrition interventions, and provide nutrition education to participants, caregivers, and InnovAge PACE staff. In-person work at InnovAge facilities or participant homes is essential to performance of duties.

Qualifications

• Bachelor’s degree in food science, nutrition, and/or dietetics from an accredited college/university is required.
• Current medical terminology and the ability to complete written assessments and document participant care.
• A minimum of one (1) year experience working with the frail or elderly is required.
• RD certification required in CA/CO/FL/NM/PA; FL requires a State Board License and additional certification within 30 days of employment.

Responsibilities

• Complete participant nutritional assessments and reassessments, screen and assess nutritional status, and plan and implement appropriate nutrition interventions.
• Educate participants, caregivers and InnovAge PACE staff on nutrition; provide home or facility-based services as needed.
• Develop and implement nutrition care plans, monitor intake and dietary modifications, and coordinate with IDT.
• Document nutrition notes in medical records and communicate diet changes to staff and caregivers.
• Contribute to Care Planning conferences and participate in interdisciplinary team activities.
