Job summary

The Center for Elders’ Independence in Oakland, CA is seeking a Home Health Nurse (RN) to deliver compassionate, patient‑centered care in clinics and in participants’ homes under the care coordination team.

You will follow individualized care plans, coordinate with an interdisciplinary team, provide health education, monitor progress, and participate in after‑hours on‑call rotations as required.

Qualifications

• Current California RN license or LVN license.

Home Health Nurse RN

The Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, and promoting autonomy, quality of life and the ability of individuals to live in their communities.

The Position: Under the direction of Manager of Care Coordination and Home Health Nursing, the Home Health Nurse (HHN) provides high-quality, compassionate, and patient-centered nursing care to participants at the CEI clinic and at their homes. This role ensures the delivery of safe, effective, and individualized care, following the participant’s care plan, physician’s orders, and regulatory standards. The HHN communicates effectively with all members of the clinic team and other program staff to ensure that participants are receiving care that is appropriate and adheres to the participants’ plan of care.

The hourly pay range for the Home Health Nurse RN position at Center for Elders’ Independence is $46.02 to $69.95. Placement within the range is determined based on job-related factors such as relevant experience, skills, abilities, internal equity, and market data, consistent with CEI’s annual compensation review and established compensation practices.

Duties and Responsibilities:

• The Registered nurse (RN) assesses participants in clinic, or in participant’s home, for post hospital, SNF, and falls assessments for medication, compliance and disease management or as indicated.
• Monitors participants for any acute physical or behavioral changes and adjusts plan of care accordingly.
• Provides skilled nursing care to participants as needed in a timely manner.
• Provides health education, information and counseling to participants, family members and care givers regarding participant’s care needs and safety.
• Implements preventative and rehabilitative nursing procedures as appropriate for the participant’s care and safety and all other reasonably related jobs as assigned.
• The RN must be available for on‑call response if needed to respond to emergency concerns.
• Must be available weekends and evenings to work and participate in after‑hours on‑call rotation as scheduled.
• In the absence of a clinic registered nurse, the Home health RN may be required to participate in IDT meetings, offering clinical expertise in team planning and problem solving in the absence of a clinic registered nurse.
• The RN conducts initial, semi-annual nursing assessments of participants in the clinic as needed to evaluate their status and support development and maintenance of individualized care plans.
• Maintains accurate and timely documentation of care provided in the electronic health record (EHR) in accordance with PACE and regulatory standards.
• Possesses strong clinical skills and experience in complex wound care, infusions, venipuncture, vaccinations, complex respiratory care, genitourinary care and ostomies highly preferred.
• Knowledge and experience in diabetic foot care and advance wound care training and/or certifications preferred.
• The RN assures quality of care by adhering to therapeutic standards, making or recommending necessary treatment adjustments, following standards of care set by the governing agency regulations.
• Participates in CEI’s quality assurance performance improvement program (QAPI) as assigned, measuring health outcomes against patient care goals and standards.
• Participates in departmental and agency meetings as directed by the supervisor and/or agency leaders.
• Maintains the confidentiality of all company procedures, results and information about participants or families.
• Complies with all agency training requirements.
• Maintains a safe working environment by following CEI’s safety P&P’s.
• Maintains a courteous, helpful and professional attitude on the job. Displays a willingness and ability to be responsive to all customer groups.
• Performs other assigned duties, demonstrating flexibility and a positive proactive approach to participant care.

Requirements:

• Must be available weekends and evenings to work and participate in on‑call rotation.
• Maintains eligibility for licensure by obtaining required continuing education units.
• Knowledge of common safety hazards and precautions to establish a safe work and living environment for participants.
• Able to lift and move participants or equipment weighing up to 50 pounds.
• Frequent standing, walking, bending, and occasional kneeling during home visits.
• Ability to react calmly and effectively in emergency situations.
• Strong verbal and written communication skills to effectively interact with participants, families and team members.
• Proficiency in using electronic health records and basic computer applications.
• Ability to work as a team player in a multi‑cultural, multi‑disciplinary setting
• Travel to participant homes and PACE centers.

Qualifications:

• Graduate from an accredited school of professional nursin g or vocational nursing program.
• Bachelor of Science in Nursing (BSN) or Associate degree in Nursing (ADN) or equivalent.
• Current California Board of Registered Nurse License or California Board of Vocational Nursing and Psychiatric Technician License.
• Current CPR - BLS certification required.
• Have a minimum of (1) year acute, skilled nursing facility or ambulatory care experience, working with the frail or elderly population.
• Must have a valid California driver’s license, motor vehicle insurance and reliable transportation.
