Job summary

The Center for Elders’ Independence in California is seeking a Clinic Registered Nurse to provide direct nursing services in the CEI clinic under the Clinic Supervisor. You will collaborate with providers and the interdisciplinary team to ensure care aligns with each participant's plan of care.

The role requires a CA RN license, at least 1 year of acute or ambulatory care experience with the elderly, CPR certification, and ongoing licensure CEUs.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current California Registered Nurse License.
• Minimum 1 year acute or ambulatory care experience with frail or elderly population.
• Current CPR certification.
• Maintains continuing education units for licensure.

Responsibilities

• Assess participants' status in person initially and as needed; assist in plan of care.
• Monitor for acute changes and adjust plan accordingly.
• Provide skilled nursing care to participants as needed.
• Provide health education and counseling to participants and families.
• Implement preventive and rehabilitative nursing procedures.
• Participate in IDT meetings and contribute clinical expertise.
• Document in medical records accurately and timely.
• Provide nursing direction when supervisor is absent; offer consultations.
• Ensure quality of care by adhering to standards; participate in QAPI.
• Attend departmental and agency meetings as directed.
