Responsibilities

• Assess residents’ psychosocial needs, strengths, goals, and barriers to care.
• Develop, implement, and update individualized care plans with the interdisciplinary team.
• Provide emotional support, counseling, and resource referrals to residents and families.
• Advocate for residents to ensure needs and preferences are communicated and respected.
• Maintain communication among residents, families, physicians, and external providers.
• Participate in care planning meetings and monitor progress as needs change.
• Coordinate discharge planning and community services to support continuity of care.
• Educate residents and families about services and transition options.
• Maintain accurate documentation and comply with facility policies and standards.
