Job summary

BoldAge PACE in New Port Richey, FL is seeking a Registered Nurse Care Team Manager to lead the Interdisciplinary Team (IDT) and coordinate participant care. This role emphasizes supervision, collaboration across disciplines, and development of comprehensive care plans in accordance with CMS and PACE standards.

You will supervise IDT staff, schedule and facilitate team meetings, and ensure high-quality, compliant care while supporting participants and their families.

Qualifications

• Minimum of 5 years working in a health care setting.
• 3 years in healthcare-related management position (preferably in a geriatric setting).
• RN license preferred.
• 1 year working with the elderly; training provided if not present.

Responsibilities

• Oversee the IDT (interdisciplinary team) and coordinate care planning.
• Facilitate IDT meetings and ensure clear communication of changes in participant care.
• Ensure documentation of care plans, interventions, and regulatory compliance.
• Provide guidance and supervision to IDT members and staff.
• Monitor and improve participant outcomes and satisfaction through quality assurance.
