Levindale Hebrew Geriatric Center, a LifeBridge Health facility in Baltimore, is seeking an experienced RN for Hall 2, a 50-bed long-term care and memory care unit. You will deliver compassionate nursing care, assess residents, and collaborate with an interdisciplinary team to promote safety, dignity, and quality of life.

The role emphasizes individualized care plans, medication administration, and documentation in EMR, with support for residents across varying acuity and needs.

Qualifications

• Active Maryland RN license is required.
• CPR certification by American Heart Association.
• Associate degree in Nursing is required.
• Strong critical thinking and patient-centered care skills.
• Proficiency with Microsoft Office and electronic medical records.

Responsibilities

• Provide safe, age-appropriate nursing care in a long-term care and memory care setting.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments while monitoring resident conditions and responses.
• Provide education and emotional support to residents and families.
• Collaborate with physicians, nursing assistants, rehabilitation staff, and interdisciplinary teams.
• Accurately document patient care and outcomes within the electronic medical record.
• Support residents with chronic conditions and dementia-related needs.
• Serve as a clinical resource and advocate for residents and families.
