Job summary

The Swinomish Indian Tribal Community seeks an experienced Heart Registered Nurse Clinical Coordinator to provide nursing assessment, care planning, and care coordination for the H.E.A.R.T. Homes program. You will support aging in place by coordinating with residents, families, and the interdisciplinary care team.

The role emphasizes clinical consultation, caregiver education, and timely communication of changes in health status within Washington state requirements and program policies.

Qualifications

• Current, active RN license in the State of Washington.
• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN); BSN preferred.
• Experience in geriatric nursing, community health, home health, or related care coordination roles.

Responsibilities

• Perform clinical assessments within scope of practice and program responsibilities.
• Contribute to individualized care plans and coordinate nursing input.
• Monitor changes in health status and risks for aging residents.
• Educate caregivers on health conditions, safety, and care plans.
• Coordinate with primary providers, specialists, and home health services.
• Maintain accurate documentation of assessments and care activities.
• Support transitions to higher levels of care and follow-up resources.
