Job summary

The Swinomish Indian Tribal Community is seeking a Heart Homes Lead Caregiver in the Medical department to provide compassionate, non‑medical elder care while guiding and mentoring fellow caregivers. You will support independence, safety, and dignity, coordinate daily activities, and communicate changes to the RN Clinical Coordinator.

You will ensure care plans are followed, promote a homelike atmosphere, and assist with meals, mobility, and wellness checks.

Qualifications

• Must have at least a high school diploma or equivalent as per policy.
• Experience caring for elders or vulnerable adults is preferred.
• CPR and First Aid certification or ability to obtain within required timeframe.
• WA long-term-care worker training orientation and competency requirements must be met.

Responsibilities

• Provide direct resident care including ADLs with dignity and safety.
• Lead shift coordination and primary caregiver resource for assigned shifts.
• Mentor and support caregivers; organize daily activities.
• Monitor resident condition and communicate changes to RN Coordinator.
• Maintain a safe, homelike environment; support meals, mobility, and wellbeing.
• Follow care plans, protect privacy, and document requirements accurately.
• Coordinate health observations, wellness checks and safety procedures.
• Support cultural responsiveness and resident autonomy in daily routines.
