Job summary

TEEMA seeks a Physician to provide direct primary medical care within the Interdisciplinary Team (IDT) framework in South Carolina. The role includes supervising PCP nurses and assistants, performing histories and physicals on new referrals, and coordinating patient care across hospital and nursing home settings.

The ideal candidate will have an MD/DO license in good standing, board certification, and at least two years of clinical experience with frail elderly populations, plus immunizations

Qualifications

• Minimum of two years of clinical experience.
• Experience or training in working with frail or elderly populations.
• Geriatric medicine knowledge and needs of complex geriatric patients.
• Medically cleared for communicable diseases and up-to-date immunizations.
• Ability to pass a criminal background check, including Medicare/Medicaid eligibility.

Responsibilities

• Provide direct primary medical care to patients and supervise medical care personnel.
• Perform comprehensive history and physical examinations on new referrals.
• Coordinate hospital admissions and discharge planning with the IDT.
• Manage care for patients residing in nursing homes with regular visits.
• Document patient changes in medical records and communicate updates to the IDT.
• Participate as a member of the IDT and attend staff meetings.
• Maintain and enhance education regarding quality clinical standards and geriatric best practices.
