Job summary

Gurwin Healthcare System in Commack, NY seeks a Per Diem Pharmacist to join our geriatric care team. You will interpret drug orders, compound IVs, dispense medications, and provide drug information to physicians and nursing staff while ensuring regulatory compliance.

Requirements include NYS Pharmacist license, hospital or LTC experience, and preferred BCGP. Weekend shifts Saturdays 8:00am–4:00pm and Sundays 8:00am–12:00pm with additional days as needed.

Qualifications

• PharmD preferred or BS in Pharmacy.
• Hospital or LTC experience required.
• BCGP (certified geriatric pharmacist) preferred.
• Current New York State license to practice pharmacy.

Responsibilities

• Interprets and enters drug orders and patient data into computer profile.
• Compounds IVs and dispenses medication upon written prescription from staff physicians, staff physicians’ assistants, and staff nurse practitioners.
• Provides drug information to medical and nursing staff.
• Supervises all functions of pharmacy technician staff.
• Reviews resident’s medical drug regimen on a monthly basis, reporting any medication irregularities, adverse drug reactions, or possible problems related to multiple drug therapy for the geriatric and making recommendations.
• Maintains computerized patient profiles for all residents.
• Dispenses medication in a manner which meets all State and Federal prescription laws,
• Demonstrates the ability to discern the medication needs and behaviors of the subacute/rehab population and to communicate to residents and/or family members relating to medication regimen.
• Maintain records and inventory of medications.
• Make recommendations to medical staff regarding formulary.
