Job summary

Gurwin Healthcare System in Commack, NY seeks a Per Diem Pharmacist to join our care-focused team and support geriatric residents across our campus. The role requires weekend availability (Saturdays 8:00am–4:00pm, Sundays 8:00am–12:00pm) and flexibility for additional shifts as needed.

Responsibilities include entering orders, IV compounding, dispensing under physician prescriptions, providing drug information, supervising pharmacy staff, and reviewing monthly regimens to optimize therapy.

Qualifications

• Pharm D preferred or BS in Pharmacy.
• Hospital or LTC experience required.
• BCGP (geriatric pharmacist) preferred.
• Current New York State License to practice pharmacy.

Responsibilities

• Interprets and enters drug orders and patient data into computer profile.
• Compounds IVs and dispenses medications upon written prescription from staff physicians.
• Provides drug information to medical and nursing staff.
• Supervises all functions of pharmacy technician staff.
• Reviews residents' medical drug regimens monthly and notes potential issues.
• Maintains computerized patient profiles for all residents.
• Dispenses medications in compliance with state and federal laws.
• Advises on formulary and communicates with residents/families about medication regimens.

The candidate will be responsible for caring for geriatric residents to ensure regulatory compliance and to continue our reputation of providing the gold standard of excellence in quality care for those we serve.

EDUCATION AND EXPERIENCE REQUIREMENTS:

• Pharm D preferred or BS in Pharmacy
• Hospital or LTC experience required
• BCGP (cerified geriatric pharmacist) preferred
• Current New York State License to practice pharmacy.

DUTIES AND RESPONSIBILITIES:

• Interprets and enters drug orders and patient data into computer profile.
• Compounds IV's and dispense medication upon written prescription from staff physicians, staff physicians' assistants, and staff nurse practitioners.
• Provides drug information to medical and nursing staff.
• Supervises all functions of pharmacy technician staff.
• Reviews resident's medical drug regimen on a monthly basis, reporting any medication irregularities, adverse drug reactions, or possible problems related to multiple drug therapy for the geriatric and making recommendations.
• Maintains computerized patient profiles for all residents.
• Dispenses medication in a manner which meets all State and Federal prescription laws,
• Demonstrates the ability to discern the medication needs and behaviors of the subacute/rehab population and to communicate to residents and/or family members relating to medication regimen.
• Maintain records and inventory of medications.
• Make recommendations to medical staff regarding formulary.
