Job summary

Eventus WholeHealth is seeking a dedicated Care Coordinator to manage patient caseloads and ensure effective navigation of primary and specialty care for geriatric clients. The role involves coordinating transitions of care, conducting medication reconciliation, and supporting timely follow-up with patients, families, and the care team.

You will collaborate with schedulers, document encounters in the EHR, and help drive quality metrics within value-based programs.

Qualifications

• LPN or certified MA with 5 years primary care experience.
• Minimum 2 years in LTC/ALF setting.
• Care coordination experience preferred.
• Value-based care experience ideal.
• Strong cultural competency with target population.
• Proficient with Excel, PCC, Matrix and Microsoft products.
• Interest in geriatric clients.

Responsibilities

• Manage client caseloads and coordinate primary and specialty care.
• Oversee transitions of care and post-visit follow-up.
• Communicate with families, POAs, and caregivers.
• Document services in patient charts and track appointments.
• Support care team to achieve quality metrics in value-based care.
