Job summary

The City of Lincoln & Lancaster County is seeking a dedicated case manager for the Geneva, NE office. You will assess needs, determine eligibility, develop personalized care plans, coordinate services, and monitor progress to help individuals thrive under Title 480 and Title 471 programs.

Desire includes a relevant degree or RN with experience; strong advocacy, problem-solving, and independence-guiding skills; valid driver's license required for service delivery.

Qualifications

• Bachelor's or Master's degree in Human Services, Education or Health fields or RN with 2 years of relevant experience.
• Experience providing case management to geriatric or disabled populations preferred.
• Two years supervisory or leadership experience preferred.

Responsibilities

• Conduct assessments of individuals to determine service needs.
• Determine eligibility for programs and supports.
• Develop personalized care plans and coordinate services across providers.
• Monitor progress and adjust plans to ensure progress toward independence.
• Advocate for clients and guide them toward greater independence.
