Job summary

CenterPointe is seeking a full-time Care Coordinator in Lincoln, NE. You will provide targeted support to individuals and families, helping navigate community resources and coordinating care during transitions.

Responsibilities include follow-up to boost engagement and ensuring coordinated systems respond to assessed needs in a timely, comprehensive manner. Monday through Friday schedule supports work-life balance with competitive benefits.

Qualifications

• Two or more years of experience in Behavioral Health.
• Knowledge of co-occurring disorders and related needs.
• At least 21 years of age and valid driver's license.

Responsibilities

• Provide targeted support to individuals receiving services.
• Assist individuals and families to navigate community resources.
• Follow-up during transitions of care to increase engagement in services.
