Job summary

Eventus WholeHealth is recruiting an RN for client caseload management in skilled nursing and assisted living facilities. The role involves coordinating care with the Mid-Level Practitioner and ICT team, performing HRAs, triaging needs, and ensuring quality patient outcomes.

Responsibilities include education of facility staff, care planning, and maintaining detailed documentation in the electronic health record. A strong geriatric focus and 2+ years LTC/ALF experience are preferred.

Qualifications

• Registered Nurse license and LTC/ALF experience preferred.
• Experience with care coordination or case management is a plus.
• Proficiency in MS Office, PCC and Matrix required.

Responsibilities

• Provide effective verbal and written communication with care teams.
• Maintain organized records and caseloads with confidentiality.
• Deliver excellent customer service and geriatric patient navigation.
• Work independently to meet deadlines and manage priorities.
• Demonstrate problem-solving with minimal direction.
• Ensure accuracy and attention to detail in all tasks.
• Develop plans of care in collaboration with ICT team.
• Authorize access to services and coordinate transitions of care.
• Deliver clinical care, evaluate progress, and communicate results to SNF staff.
• Oversee hospital discharge transitions and post-discharge follow-up.
• Collect labs and reports and review with medical leadership.
• Document Advance Directives and care decisions.
