Trinity Health IHA Medical Group is seeking a compassionate Care Manager to join our outpatient team in Michigan. You will support patients through clinical triage, care coordination, and ongoing education to improve outcomes and patient experience.

In this role, you will ensure seamless transitions of care, collaborate with the care team, and provide targeted interventions to avoid hospitalizations, while enjoying a day shift in a full-time onsite position.

Qualificações

• RN licensed and trained to coordinate care
• Experience in oncology/primary/ambulatory care preferred
• Knowledge of chronic conditions, prevention, wellness and risk assessment

Responsabilidades

• Collaborates with the health care team to deliver quality, patient-centered and cost-effective care
• Provides self-management support and patient education to at-risk patients
• Triages patients and ensures appropriate delivery of care per protocols
• Plans, implements, monitors, and evaluates individualized patient care to optimize health
• Assists with transitions of care and readmission prevention
• Coordinates care across the continuum with the health care team to optimize outcomes
