Ambulatory RN Care Manager: Day Shift, No Weekends

Job summary

Trinity Health Alliance of Michigan is seeking an experienced RN Care Manager to join the primary care team in a full-time, day shift. The role focuses on high-risk members, assessment, planning, and coordination across the care continuum to prevent readmissions and ensure comprehensive, patient-centered care.

You will work closely with physicians, nurses, and care managers, performing health risk assessments, developing outcome-based plans, and documenting care in the electronic medical record.

Qualifications

• Graduation from an accredited BSN Program.
• Current Michigan RN license, CPR certification, valid Michigan driver’s license.
• Three years acute care or case management experience.
• Preferred: Two years of utilization or HMO experience.
• Fast paced environment, managing multiple tasks and patients at the same time.
• Possible exposure to infectious disease, hazardous chemicals, and blood and body fluids.
• Ability to work flexible schedules.

Responsibilities

• Keeps member/family informed and requests further assistance when needed.
• Demonstrates ability to follow through with requests, sharing of critical information, and timely communication.
• Functions as liaison between administration, members, physicians and other healthcare providers.
• Interacts professionally with member/family/physicians and involves them in plan of care.
• Performs a Health Risk Assessment (HRA) and determines acuity score for follow-up.
• Develops and implements outcome-based care plans and documents in the medical record.
• Maintains NCQA-aligned workflows and manages caseload to meet contract goals.
