Job summary

ADP, Inc. is seeking a Care Manager in Englewood, NJ to deliver grant-funded care management services for older adults.

The role includes in-home and telephone support, needs assessments, and developing individualized care plans to improve participant well-being. The position requires an Associate's or Bachelor's degree in Social Work (preferred), minimum two years of relevant experience, a valid NJ driver's license, and the ability to pass background checks.

Qualifications

• Associate's or Bachelor's degree in Social Work or related field (preferred).
• Minimum of two years' experience working with older adults preferred.
• Experience with community-based or grant-funded programs a plus.
• Valid NJ driver's license required.
• Ability to pass all required background checks.
• Ability to work a flexible schedule to meet participant needs.

Responsibilities

• Perform regular face-to-face home visits with clients.
• Develop and implement a care plan and goals based on assessment and client's needs.
• Evaluate and update care plan on a regular basis.
• Communicate regularly with family members and related professionals.
• Cultivate and maintain knowledge of providers and community resources, and link clients to appropriate services.
• Document assessment, ongoing progress, and all services provided in data system.
