Job summary

Bergen Family Center is seeking a Part-Time Care Manager – Social Isolation Grant to deliver individualized care management to older adults in Englewood, NJ. The role includes home visits, telephone outreach, and documentation to meet grant requirements.

You will assess, plan, and coordinate services, connect participants with resources, and advocate for independence while ensuring compliance and reporting for the grant-funded initiative.

Qualifications

• Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Gerontology, or a related field preferred.

Responsibilities

• Conduct comprehensive in-home assessments to identify participant needs, strengths, risks, and barriers to engagement.
• Develop, implement, monitor, and update individualized care plans based on participant goals.
• Conduct regular in-home visits and telephone outreach to reduce social isolation and promote participant well-being.
• Monitor participant progress and adjust care plans as needed.
• Connect participants with community resources, healthcare providers, support services, and benefit programs.
• Identify unmet needs and coordinate appropriate referrals to enhance participant independence and quality of life.
