Job summary

Appalachian Mountain Health is seeking a Licensed Practical Nurse (LPN) to join our Federally Qualified Health Center outpatient team. You will assist with patient assessments, implement care plans, and provide education under supervision of a Practice Supervisor and RN.

Maintain high-quality patient care in a collaborative setting. You will support data collection, nursing interventions, and ensure compliance with nursing standards and confidentiality across diverse patient populations served

Qualifications

• Knowledge of LPN scope of practice in outpatient settings.
• Ability to perform annual wellness visits and preventive care.
• Proficiency in basic nursing procedures, vital signs, and physical assessments.
• Proficiency with electronic medical records and healthcare technologies.
• Knowledge of North Carolina Nursing laws and LPN rules.

Responsibilities

• Assist with patient assessments, data collection, and nursing history.
• Support care planning and contribute to patient care plan development.
• Implement nursing interventions and administer orders per plan.
• Delegate appropriate tasks to unlicensed staff with supervision.
• Educate patients and families on health conditions and self-care.
