Job summary

Atlantic Health is seeking a Licensed Practical Nurse to provide patient-focused care under supervision of an RN. You will deliver care per the plan, document and communicate findings, and contribute to safety and quality across the care team.

Responsibilities include collaboration with clinicians, caregiver education, timely documentation, and supporting agency processes. A valid driver’s license and reliable vehicle are required for home visits.

Responsibilities

• Delivers safe care according to plan of care and as delegated by the RN.
• Collaborates with health care team and reports findings to RN or designee.
• Assumes responsibility for self development and maintains competencies.
• Effective communication with clinical staff to facilitate care planning.
• Assists RN in patient/family communication and provides written teaching materials.
• Documents all communication timely with teams.
• Assists in growth of the agency and accepts other assignments as needed.
• Supports patient safety by reporting occurrences and teaching safety measures.
• Assists in development of agency processes and provides orientation to new employees.
