Qualifications

• Provides patient management, care coordination, and discharge planning for inpatient and outpatient settings.
• Assists Veterans, family members, and caregivers with receiving appropriate options and services.
• Coordinates care with multiple providers across all levels and sites of care.
• Addresses psychosocial, nursing and medical needs of patients and families/caregivers; participates in interdisciplinary care management.

Responsibilities

• Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
• Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
• Coordinates care with multiple providers across all levels and sites of care.
• Collaborates in the development, implementation, evaluation of policies and guidelines.
• Demonstrates leadership to improve care for a group of patients and supports colleagues through knowledge sharing.
