The Veterans Health Administration in Florida is seeking an experienced Interventional and Specialty Care Coordinator RN to provide patient management, care coordination, and discharge planning for inpatient and outpatient settings.

You will collaborate with the interdisciplinary team to deliver education, optimize outcomes, administer medications, and ensure timely access to services for Veterans. Monday–Friday schedule, not eligible for telework, with competitive federal benefits.

Qualifikationen

• English language proficiency is required.
• Eligible for RN licensure in a U.S. state.
• Graduate of a nursing program approved by ACEN/CCNE.
• Bridge program in nursing may allow RN licensure before completion.
• State license obtained upon graduation and temporary appointment process.
• Foreign nursing education requires degree equivalency validation.
• Current, full, active, unrestricted RN license in the U.S.
• GNT exception: temporary appointment while pursuing licensure.

• Care coordination for inpatient and outpatient Veterans care.
• Collaborate with interdisciplinary team for planning and discharge.
• Administer medications and monitor patient responses.
• Review labs and imaging orders and provide information to providers.
• Engage in shared governance and quality improvement efforts.
• Educate patients and families to support self-management and outcomes.
