Job summary

Old Colony YMCA seeks a dedicated Geriatric Support Service Coordinator (GSSC) to join our Senior Care Options team in the South Shore area. You will determine eligibility and ensure access to appropriate services for eligible members.

Ideal candidates have at least two years in human services, a Bachelor's in social work or related field, licensure if appropriate, and strong written/verbal skills. Bilingual ability (Cantonese or Haitian Creole) is highly preferred; hybrid work model offered.

Qualifications

• Bachelor's degree in social work or related field preferred; licensure if applicable.
• Two years of experience in human services, preferably with elder population.
• Bilingual ability preferred (Cantonese or Haitian Creole).
• Excellent written and verbal communication; proficient with computers.

Responsibilities

• Promote independent functioning and coordinate services in the least restrictive environment.
• Perform initial and ongoing assessments and develop individualized care plans.
• Develop rapport with consumers to facilitate coordination, monitoring, and adjusting care plans.
• Provide regular contact with consumers, including home visits, to ensure progress toward goals.
• Maintain accurate documentation.
