Carease Health is seeking a compassionate Care Companion/Social Worker to join our Collaborative Care team. You will visit skilled nursing facilities to assess residents’ emotional and psychosocial needs, provide brief therapy, and document care interactions.

You'll collaborate with psychiatric professionals, participate in monthly clinical meetings, and benefit from paid training and ongoing supervision. The role emphasizes resident-centered care and professional growth within a supportive team.

Qualifikationen

• Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• 1 year of experience.

Aufgaben

• Visit designated facilities to assess residents’ emotional, cognitive, and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain detailed records after each visit.
• Collaborate with psychiatric professionals and participate in monthly meetings with the psychiatric NP.
