Partners in Care Foundation is seeking a qualified Case Manager to support elderly clients in their homes across California. The role emphasizes comprehensive psychosocial assessment, care planning, and coordination with health professionals.

The ideal candidate holds a bachelor's degree in a relevant field, at least two years of geriatric case management experience, and a proven ability to work with multidisciplinary teams to maintain client independence and well-being.

Qualifications

• Bachelor's degree in social work, psychology, counseling, rehabilitation, gerontology or sociology.
• Two years of experience working with the elderly.
• Two years of case management experience.
• Experience conducting home visits with clients.
• Knowledge of aging and programs for seniors; ability to work with multidisciplinary teams.

Responsibilities

• Evaluate the client as a total person and identify functional limitations.
• Perform in-depth psychosocial, rehabilitation and environmental assessments.
• Refer clients to specialists for detailed evaluations and recommendations.
• Develop and monitor the care plan and services.
• Coordinate with health practitioners and document case records.
