All Seniors Foundation in Los Angeles seeks an experienced Geriatric Care Manager to oversee and coordinate comprehensive home-based care plans for elderly clients. You will collaborate with families, physicians, nurses, social workers, and community partners to ensure seniors receive person-centered support while maintaining independence at home.

The role focuses on developing tailored care plans, navigating resources and insurance, coordinating services, and monitoring progress.

Qualifications

• Bachelor’s degree in Social Work, Nursing, Gerontology, or related field preferred.
• Experience in care management, home health, or eldercare coordination.
• Strong communication, organizational, and problem-solving abilities.
• Knowledge of local community resources and healthcare systems.
