Qualifications

• Develops and documents plans of care with goals in collaboration with the interprofessional team.

Responsibilities

• Assess patient needs and plan care with the care team.
• Deliver nursing care to adults 55+ in a behavioral health setting.
• Communicate care plans across the continuum of care.
