Responsibilities include comprehensive nursing assessments, implementing care plans, and collaborating with a multidisciplinary team to achieve optimal patient outcomes.

Qualifications

• Direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care.
• Safety-sensitive position with need for constant alertness and safe practices.
• Collaborates with interprofessional team to implement and adjust patient plans of care.

Responsibilities

• Promotes patient and family centered care in a healing environment.
• Educates patients and families on illness management, post-treatment home care needs and medication administration.
• Participates in patient safety, quality, and regulatory compliance activities.
• Develops, implements, and documents plans of care with defined goals in collaboration with the interprofessional team and patient/family.
• Uses nursing skills to assess physical, psychosocial, and emotional needs and communicates effectively across the continuum of care.
