Job summary

Midland Care Connection seeks a Nurse Practitioner to deliver patient-centered primary care to older adults in rehabilitation centers, skilled nursing facilities, assisted living communities, and independent living settings. The NP will assess, manage, and coordinate care with a multidisciplinary team, supporting dementia and geriatric needs.

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• Current APRN licensure and board certification.
• Prescriptive authority in Kansas.

Responsibilities

• Provide comprehensive primary care to older adults in residential and long-term care settings.
• Develop individualized treatment plans and manage acute and chronic conditions.
• Order and interpret diagnostic tests; monitor patient health.
• Coordinate care with physicians, nurses, hospice, home health and rehabilitation teams.
• Assist families in understanding treatment options and goals of care.
