Job summary

Element Care is seeking an Advanced Practice Provider (APP) – NP/PA to join the PACE Interdisciplinary Team providing comprehensive primary care to participants. The role includes annual and semi-annual exams, home and facility visits, after-hours on-call coverage, and patient education, with emphasis on timely documentation and budget-conscious care.

Requirements: MSN or PA Studies, Massachusetts NP/PA certification, MA license and DEA privileges, CPR, and at least 2 years of geriatric

Qualifications

• MSN or PA Studies degree required.
• Current Massachusetts NP/PA board certification.
• Massachusetts and DEA prescription privileges.
• Credentialed for Element Care LTC facilities.
• Current CPR certification.
• Minimum 2 years working with geriatric population.
• Clear and succinct communication with all employees.
• Ability to lead independently and work well in a team.
• Detail-oriented, self-starter, and timely task completion.
• Ability to anticipate issues and implement effective solutions.

Responsibilities

• Participates on the PACE Interdisciplinary Team to plan and evaluate care plans.
• Conducts annual and semi-annual physical examinations.
• Evaluates participants in homes, ALFs, centers for episodic visits and follow-ups.
• Performs nursing home and home visits as clinically indicated.
• Provides health care teaching to patients, families, and caregivers.
• Maintains accurate, timely documentation and adheres to budgetary constraints.
• Travels locally to participant sites as needed.
