Job summary

Xpress Wellness Urgent Care is seeking an Advanced Practice Provider (NP/PA) to deliver high-quality care to elderly and medically complex residents in a long-term care facility. The role emphasizes collaborative practice with physicians, nursing staff, and other clinicians to manage chronic conditions and promote overall well-being.

The AP Provider will perform rounds, coordinate care in the EHR, and respond to orders and after-hours inquiries.

Qualifications

• Current NP or PA license required.
• Board certification as NP or PA.
• State and federal licensure including DEA where applicable.
• BLS certification.
• Knowledge of OSHA, HIPAA and related regulations.
• Excellent communication and interpersonal skills.
• Ability to work efficiently in a fast-paced environment.
• Experience with electronic medical records.
• Understanding of medical coding and billing.
• Strong decision-making and problem-solving abilities.

Responsibilities

• Perform rounds at the facility including taking patient history, exam and documenting findings in the EHR.
• Organize rounding schedules, notes, labs and imaging in the EHR for patients seen.
• Coordinate with Director of Nursing and floor nurses for concerns and relay orders during rounds.
• Serve as first point of contact for nurses with questions; occasional after-hours calls/texts.
• Document findings and treatment plans with follow-up notes in the patient chart.
• Manage special projects and duties as assigned; this is a safety-sensitive position.
