Job summary

Xpress Wellness Urgent Care is seeking a skilled Advanced Practice Provider (NP/PA) to join our team serving elderly and medically complex residents in a long-term care facility. The provider will work with physicians and nursing staff to deliver comprehensive, high-quality care and manage chronic conditions.

Responsibilities include rounds, ordering and reviewing labs, communicating orders, documenting findings, and participating in special projects.

Qualifications

• Current Nurse Practitioner or Physician Assistant license.
• Board certification Nurse Practitioner or Physician Assistant.
• State and federal licensure including DEA.
• BLS certification.
• Knowledge of OSHA, HIPAA, bloodborne pathogens and others.
• Excellent communication and interpersonal skills.
• Ability to work in a fast-paced, high-stress environment.
• Experience with electronic medical records systems.

Responsibilities

• Perform rounds in the facility, taking history and exams, document findings in the EMR.
• Organize rounding schedule, notes, labs, and imaging in the EHR.
• Communicate with Director of Nursing before and after rounds to relay orders.
• Serve as first point of contact for nurses with patient questions; occasional after-hours contact.
• Notate findings and treatment plans in the chart, including follow-up notes.
• Manage special projects and duties as assigned; safety-sensitive position.
